Muscle Dreams Eligibility Criteria
Muscle Dreams ELIGIBILITY CRITERIA
Any child or young person aged between 8 to 28 years with muscular dystrophy and who has not been the recipient of a Muscle Dream either through MHF or via a similar organisation and/or a registered charity
Nominations for Muscle Dreams can be made by ~
- Any parent and/or guardian, grandparent and/or siblings
- School teachers and/or family friends
- Medical practitioners ie. GP, consultant, nurse, social worker
- If you are 18yrs or over, you may nominate yourself
The NINE STEPS after a nomination has been received
1. MHF Trustees will decide every 3 months which Muscle Dreams will be fulfiled
2. MHF will contact the person who made the initial nomination
3. A medical declaration release form will need to be completed by the beneficiary
4. The beneficiaries medical practitioner will be contacted
5. Medical eligibility will be established before any planning begins
6. Muscle Dream details will be thoroughly discussed with each family
7. Profile of beneficiary and expectations will be ascertained
8. Photographic and media/PR permissions will be sought with each family
9. Each Muscle Dream will have its own dedicated file
